This device is an Edimax BR-6675nD. It's board is
somewhat similar to the one used by the BR-6475nD.

Per the manual, this device has two SSIDs:
Hawking_HD45R_2.4GHz and Hawking_HD45R_5GHz.

There are a few entries (6) for the 2.4GHz SSID
in WiGLE as of 03/30/2013, all with 00:0E:3B OUIs.
