"21518D1560A11A1" and "REV: A1" is silkscreened on the board in the FCC photos. It would seem that Cameo is the probable manufacturer of this device.

The certification states the device is three-stream for both bands but it is most likely four-stream since D-Link claims 600Mbps speeds and non-standard 256-QAM was not found in 802.11n at the time that this was released.
