Overview

This device is clearly a Cameo WLG-1202.

"NETGEAR" and "32-bit Wireless PCI Adapter Rev.A3" (as well as a large bit of FCC compliance text, some other stuff, the FCC ID, and the IC ID) is silkscreened on the board. A label on the back of the board identifies the device as a WPN311 V1H2.

This device is presumed to use AR2414 based on the specifications. The FCC pictures of the card also indicate that is indeed an AR2414.
