"P/N:141000000017W" and "REV:R0B" is silkscreened on the back of the board in the FCC photos.

A label affixed to the back (.. and the schematics) notes the AWB (Accton Wireless Broadband) model as WUS620-H-JET

This device would appear to be using a Skyworks (SiGe) SE2576L power amplifier IC.
