This device is known as Connect Box, distributed by UPC (Liberty Global) within Europe.
User Manual

"P/N: 1417465200000" and "MODEL: CH7465CE" is silkscreened on the board (OEM: Arcadyan)

The default SSID and credentials are printed on a sticker on the router's chassis.

The format is not known and will probably depend on the target market / rebrand type.
